  • Final Hospital Bills,
  • Should have a Statement of account from the Hospital,
  • Doctor’s Prescription
  • Must have a Treatment protocol form the attending Physician.

  • Laboratory Protocol
  • Doctor Order and
  • Laboratory Request.

  • Funeral Contract Showing Outstanding Balance of the Service
    • With Name and Signature of the Funeral’s Delegate.
  • Registered Death Certificate With Registry Number

  • Certificate of Indigency of the Client
    • Valid ID of the Client Showing the Same address with the Barangay indigency
  • Permit to Transfer
